At Lincoln Family Medicine Center, we're committed to making your experience as smooth and supportive as possible, both in the clinic and online. On March 23, 2026, we will transition to a new electronic health record system, Epic. We believe it will make a real difference by helping patients and providers stay connected, making healthcare more convenient and accessible. With Epic, you will access MyChart through our patient portal on our website.

Epic’s MyChart allows you to do more than access your health records––you can communicate directly with your providers about your care, and link all of your healthcare accounts and access them using one login. It’s comforting to know that you can manage all aspects of your healthcare securely. 

Here are some of the benefits you can expect with MyChart.

1. Access to Health Information

Your health information is an important part of your care. It tells the story of your health over time. When you can easily access it, you and your provider can make informed decisions together. It helps your care team know when to recommend screenings, immunizations, and annual exams, and it can highlight patterns such as family history of illnesses or chronic diseases that may increase your risk. Staying connected to your health information means you’re actively engaged in your care.

2. Communication with Providers, Across all Specialties

Often, after a visit with your provider, it’s easy to think of questions later or wish you had written down everything that was discussed. With MyChart, you can revisit your provider’s notes anytime to review what was discussed, the plan moving forward, and any recommended treatments. When you have questions, there’s two-way communication between you and your provider without calling the clinic.

MyChart also improves communication across your care team. When multiple providers or specialists are involved, they can securely share information and stay aligned on your care. This connected approach helps ensure nothing is missed and you are receiving coordinated, seamless support.

3. Tools to Keep You Healthy

It’s important to be proactive in your health. MyChart gives you tools to do that. Features like symptom checkers and health questionnaires help you track changes and share important information with your providers. You can connect to your devices, such as blood pressure monitors and fitness trackers. This way your care team has a complete understanding of your health, and you feel like you are making informed decisions about your care.

4. Patient Education Resources

Easy access to patient education is key to staying healthy. When you understand your conditions, treatments, and preventive care options, you follow your care plan more confidently. MyChart provides educational resources tailored to your health needs so you can have an active role in learning about medications, lifestyle changes, and screenings.

5. Billing and Payments

After a visit, it can sometimes take weeks before you receive an insurance provider’s explanation of benefits or a clinic statement. With MyChart, you can view bills, check insurance information, and make payments or set up plans, all in one convenient and secure place.

During the changeover, you may not be able to access the patient portal. We appreciate your patience and understanding as we make this transition to MyChart. If you have questions, our team is here to help.

When we transition to Epic, all of our patient information will no longer be accessible from Athena, our current electronic health record. Patients will need to update their medical history and intake forms. This will ensure that we have all of your current health information in the new system.
